Complete patient evaluations and physical exams with thorough, timely documentation to include coordination of care. Patient follow-up visits regarding patients' progress with medication and/or treatment. Contact patient's PCP and/or specialist(s) when necessary. Evaluate lab results and provide consultation with Mid-Level Providers regarding abnormal results. Review and oversee progress and treatment plans, including making any adjustments
